Osteotomy around the knee joint is a surgical technique aimed at correcting metaphyseal bone deformities of the distal femur or proximal tibia in order to restore the physiological alignment of the mechanical axis of the lower limb. Realignment improves load distribution, enhances knee biomechanics, and slows the progression of osteoarthritis, making this procedure particularly suitable for younger and active patients. After a period of decline following the introduction of total knee arthroplasty, knee osteotomy has experienced renewed interest in recent decades due to advances in surgical techniques, modern fixation methods, and an improved understanding of knee biomechanics. Successful outcomes depend on careful patient selection, detailed preoperative planning, and the choice of the appropriate type and site of osteotomy. When performed correctly, knee osteotomy preserves the native joint, improves quality of life, and can significantly delay or even eliminate the need for total knee arthroplasty.
